If its just the N45AP you are looking for, I created one which will show your springboard in this order (should show the phone and SMS but haven't tried):

    Dock:
    Music
    Video
    Photos
    Safari

    Springboard:
    Phone
    SMS
    Mail
    Weather
    Notes
    Maps
    YouTube
    Calendar
    Address Book
    Clock
    Calculator
    Preferences
    Installer
    SSH

    Hiden:
    Stocks
    iTunes Store
    (Springboard
    DemoApp - default hidden
    )

Extract 'N45AP.plist.zip' and then upload the N45AP.plist file to:
    /System/Library/CoreServices/SpringBoard.app/

    Attached Files:

Not this one.. It won't display Phone and SMS apps..

All we need is a plist editor for Windows =)..

    Use MobileTextEdit!!

You don't need an editor for Windows! Navigate to the *.plist file in MobileFinder. If you have MobileTextEdit installed, you can click on the plist files and edit them on the touch. I did find this link in searching for a plist editor, but I don't know if it works...
