If you have Cydia, you're jailbroken. Cydia requires Internet, although I'm not sure if it needs a connection to load. Themes are managed with an application called WinterBoard.

I'm not up-to-date on the "must-have" applications and themes, so I can't help you there.

Winterboard is compatible with any device at the moment.
You can update, but you will lose your jailbreak and will have to re-jailbreak with redsn0w.

    Yep, don't update.
    3.1.3 isn't worth the trouble.

Make sure you have an internet connection, and start up Cydia.
It will start loading and probably ask to update, just do a full update (might take a while).
Then look some themes up on the internet, and search for them in cydia.
If you don't have winterboard already installed, it will automatically when you download a theme.
If after that it still doesn't show an app called winterboard on your springboard (the menu with all the icons) try looking for an app called poof (which can hide certain icons from your springboard) and make sure winterboard isn't hidden.

With winterboard you can enable custom themes.

Beware Winterboard may slow down your iPod.

Nothing else besides winterboard. You could edit some elements of the UI, though, via SSH without winterboard.
