So I got an app called icall and it is a voip calling app. Well when I got they made me register for an account, I did so. Then they made me call a number to register from my cell phone. I called it and it said I'm registered. Then I called a phone with the ap and it said my cell phone number is calling, will this cost extra on my tmobile cell phone bill to use this app?

No, it wont. iCall just wants to use your phone number to assign to your device. then, its simply duplicated. T-mobile can't and won't charge you anything extra.
