Your iPod automatically syncs with iTunes every time you plug it in.

    Is that considered a backup?

I don't use iTunes much to manage my iPod, but I'm pretty sure a backup is just an image of your old iPod, telling you what you synced such as Podcasts, images, etc. Once again I'm not sure but I think iTunes automatically backs up your iPod as long as you have it configured to do so.

No, if you restore your iPod it will ask if you want to make a backup of it. Once it is cleared, it will then ask if you want to load the backup, so you won't have to re add passwords and app settings.
