I'll be the first to say that I don't think it should be included. It doesn't make sense for a new device to include an accessory that isn't needed for all users of that device. It's only for compatibility, so it's not required.

Think of all the people that would just throw it in the drawer... or sell it.

I think for the first generation of devices that are released with the new dock connector the adapter should be free. But I think by the time that the next gen devices are out there will be more accessories that will already include the new dock connecter so after that Apple might aswell change a few quid for them.

If they are not included they should at least charge something closer to what they cost to produce and ship. Apple would never do such a thing though, going on past experiance.

I think the cable should be included with any new accessories for free or at least a 50% price reduction. For example if you choose to buy a current iPhone dock that still has the 30 pin connector, then Apple should include the adapter using the guidelines above.

It makes sense for many reasons.
    1. Apple can move older merchandise stock
    2. Apple gets to be green by not wasting that stock
    3. Customer gets the new cable for their new to them accessories
    4. Similar to providing Latest version of OS X to new Mac purchase
    5. Entices customers to buy an accessory they weren't sure about getting before. Could make the difference to buy.

The only caveat I would apply to this though is that the customer should have to get the accessory from Apple, whether online or in store.

They shouldn't have to give new dock connectors away at every store that sells iOS accessories.

Apple should give at least one free with every new device

Lots of people will be unhappy to buy their new iPhone 5 and find out they can't use it with their old docks until they return to the Apple store to by an adaptor which costs peanuts for Apple to produce

Wait, you think Apple will give something away for free?

No. IT will not be free. Probably $10 or $15.

They have before, like the bumber cases. I think there should be maybe a mail-in rebate for people to get it for free. I just hate the fact that if i buy the new iPhone i will not be able to use all of my accessories (4+) with it without spending 40 - 60 bucks.

So you are saying they shouldn't innovate? Changes need to happen. If a company hasn't changed anything in many years, they are doing something wrong.

They can and should innovate, but they don't have to do it quickly. I mean they can give out rebates for a couple months, and that wouldn't be hindering their innovation at all. Just until manufacturers catch up with Apple.
