When Apple plans their keynotes, how far back do they come up with their ideas for devices and start creating them? Just curious...

I don't entirely understand what you are asking, But most keynotes are about 1-3 weeks before an iPod, iPhone, or iOS Update's release. They manufacture their devices ages before the keynote, how do you think Gizmodo got their hands on the iPhone 4 and later faced a crapload of trouble for it ?

Years. The iPad came before the iPhone (in a way).
