iTunes hasn't been accepting the he .mp4's I've been converting; what are the exact specifications for the .mp4 video and audio type it requires? Are there any other requirements?

If you want to go for H.264, you have to deal with these restrictions:
    -Baseline Profile
    -max. AVC Level 3
    -max. resolution of 640x480
    -max bitrate of 2.5Mbps
    -No cabac, b-frames...

Besides H.264, Mpeg4-SP would also be an option, but H.264 would probably deliver a better quality.

If you use Handbrake (a great free video converter), it has a preset for the iPod touch and iPhone; it will apply the appropriate requirements automatically.

Or even better, iSquint or VisulaHub which will convert them and add them straight to iTunes
